If you have recently scheduled to see me, I want to welcome you to my practice.

I will make every effort to help you feel comfortable as we work together toward resolution of the issues that brought you to me.

Patient Registration

We normally ask new patients to arrive for their first visit about 10-15 minutes early to complete our registration.

If you prefer, however, you may complete the registration in advance and bring the documents with you.  (For any patients under 18 years old, both the patient and a parent or guardian must sign.)

Please note that you will also need to bring with you a photo ID and insurance card.

The four documents are listed below with links for you to access and print:

Registration – Please complete the Registration and bring it with you to your first appointment.

Client Services Agreement – Please review the Agreement, sign and date it, and bring it with you to your first appointment.

Credit Card Billing Authorization – Please complete the Authorization if you want to use a credit or debit card for your co-payment, co-insurance, deductible and any other patient obligations.  Our billing agent accepts Master Card and Visa.

Notice of Privacy Practices under HIPAA – Please review this Notice and keep a copy if you wish.  You need not bring it with you.
